Review your medical records to confirm that the services billed are accurate and consistent with your medical record.

Advance Care Planning

This process supports adults of any age or stage of health in understanding and sharing their personal values, life goals, and preferences regarding future medical care so they receive medical care that is consistent with their values, goals, and preferences during serious and chronic illness (Sudore, et al., 2017)
As your advocate, we will assist you in the process of developing your Advance Care Planning (ACP) document(s).
